The Health Records Clerk assists in the establishment and maintenance of health records in accordance with policies and procedures. The clerk is responsible for the daily management of the patient record including filing, photocopying and scanning of patient records, chart location, chart pulls, chart assembly and chart distribution. The Clerk also provides clerical support for Release of Information (ROI) and quality assurance (QA).
Rate of Pay: $24.36 – $26.36 per hour
Hours of Work: 37.50 hours per week

Qualifications
- Successful Completion of a Health Records Clerk Certificate , Health Care Office Assistant Certificate or an equivalent combination of education, training and experience as
- determined by the Hospital
- Demonstrated proficient knowledge of Microsoft Office (Word and Excel), and other corporate productivity tools
- Demonstrated basic knowledge and understanding of medico-legal aspects of chart completion as required by the Public Hospitalโs Act
- Demonstrated proficient knowledge of LHSC policies, i.e. patient confidentiality and privacy legislation
- Demonstrated proven excellence in customer service
- Demonstrated proven attention to detail
- Demonstrated problem solving and decision making skills
- Demonstrated effective organizational skills involving time management and prioritization
- Demonstrated effective communication (both written and verbal) and interpersonal skills preferred
- Demonstrated practice and commitment to the principles of patient and family centered care
- Demonstrated practice and commitment to patient and staff safety at LHSC
- Demonstrated practice and commitment to LHSCโs vision, purpose and values
- Demonstrated ability to attend work on a regular basisImmunization Requirements
Before beginning employment at LHSC, all new hires will be required to:
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis;
- Provide documentation of the Tuberculosis skin testing (two step);
- Provide proof of Health Canada-approved COVID-19 vaccination reflective of Ontario Healthโs current definition of being fully vaccinated
