This opportunity is for Community based work in areas of Somerset, responding to urgent care in the patients place of residence. There are options to work from South Petherton Community Hospital or Harrison House in Taunton. You tell us your preference!

We value our RGNs highly, so development and training within this role will be offered and prioritised. There is an expectation that hours are worked flexibly over a rota basis which does include evenings and weekends. We have full time hours available.

You will require good communication skills, a team work approach and be able provide a high standard of nursing care with the support of your team.

Main duties, tasks & skills required

Rapid Response is a service which aims to meet patients’ urgent care needs at home by providing urgent clinical, therapy or support interventions to keep a patient in their place of residence, which is key in improving patient outcomes, preventing avoidable hospital admissions and delivering NHS strategic priorities.

The service is part of the new Urgent Community Response agenda and will respond within two hours of a referral to meet the UCR national guidelines.

The RGN will provide and maintain a high standard of evidence based, patient centred care. To demonstrate good interpersonal skills and to develop practical skills required within the speciality, whist reflecting the philosophy of care.

To deliver patient care on a daily basis, using the systems in place in the department for a multi-disciplinary approach to patient-centred care delivery, which builds on the philosophy of greater involvement of patients and their relatives/carers by using the Trusts patient and public involvement strategy.

Experienced Band 5 post holders will also provide effective clinical and managerial leadership of the department, in the absence of the senior members of the team, which empowers staff to deliver patient- centred, evidence based nursing care
