This post is a rewarding and exciting opportunity to help transform the delivery of community-based services. The development of a new integrated service model across health and social care is a key priority for North Yorkshire place. There is a clear ambition from all partners to develop community services that work across boundaries and improve the health of their population.
This vision includes a significant ambition to radically reshape the delivery of intermediate care and develop schemes and services through the Better Care Fund. This includes building capacity to prevent avoidable admissions through preventative work and crisis response services, combined with delivering a Home First ethos following discharge from hospital to improve longer term outcomes.
As a senior service improvement manager, you will play a leading role in turning this exciting vision become a reality. With the support of a place-based team, you will take forward the transformation of key local services as part of the North Yorkshire integrated health and social care model. You will forge strong partnerships with community provider organisations across North Yorkshire, North Yorkshire Council and the voluntary sector, helping the system to work together to deliver shared goals.
